Notice of Data Incident

Posted: April 15, 2025

On June 25, 2024, Minyard | Morris LLP observed unusual activity on its computer network and immediately began an investigation with our IT team and third-party specialists. The investigation determined that certain files on our network were potentially accessed without authorization between June 16 and June 25, 2024. Therefore, we began a comprehensive analysis of the files at issue to determine the specific information contained therein and to whom the information related. We also engaged third-party data review specialists to assist our analysis. The third-party data review recently concluded and determined the files at issue may include names of individuals in combination with one or more of the following: Social Security number, driver’s license number, financial account information, passport number, taxpayer ID number, and medical treatment/health insurance information.

In response to this incident, we worked with third-party specialists to investigate and implement additional security measures. We also notified law enforcement, and we are reviewing our policies and procedures related to data protection. We have no reason to believe any information has been or will be misused because of this incident. However, if you have questions about this incident or believe you may be affected and would like to enroll in complimentary credit monitoring and identity protection services, please call 1-833-799-3871 between 8:00 a.m. to 8:00 p.m. Eastern, Monday through Friday, excluding holidays. You may also write to us at 1811 Quail Street, Newport Beach, CA 92660.

In general, we encourage individuals to remain vigilant against incidents of identity theft and fraud by reviewing credit reports/account statements and explanation of benefits forms for suspicious activity and to detect errors. Under U.S. law, individuals are entitled to one free credit report annually from each of the three major credit reporting bureaus, TransUnion, Experian, and Equifax. To order your free credit report, visit www.annualcreditreport.com or call 1-877-322-8228.

Individuals have the right to place an initial or extended fraud alert on a credit file at no cost. If individuals are a victim of identity theft, they are entitled to an extended fraud alert lasting seven years. As an alternative to a fraud alert, they have the right to place a credit freeze on a credit report. The credit freeze is designed to prevent credit, loans, and services from being approved without consent. Pursuant to federal law, individuals cannot be charged to place or lift a credit freeze on your credit report.

Should you wish to place a fraud alert or credit freeze, please contact the three major credit reporting bureaus listed below:
